Your Time in Quarantine & Isolation

We thank you for your cooperation during your time in Quarantine or Isolation (Q/I).

For Everyone Entering Q/I:

  • Review the Q/I Compact.
  • It is vital that University Health Services (UHS) is aware of any underlying health conditions you may have. If you have an underlying health condition that may be exacerbated by COVID-19, complete this form to notify a UHS Clinical Case Manager, who will be reaching out to discuss. If you are not aware of any underlying health conditions, you do not need to submit a form.
  • You will receive a call from the Care and Concern team within the first 24 hours of your Q/I stay. Care and Concern team will reach out regularly to students following the initial outreach. You can also reach out to them directly anytime at 574.634.1148 or covidcc@nd.edu
  • Review the information in your packet, and on this page, and contact the Care and Concern team with any questions you may have.

For Students in University-sponsored Q/I Spaces:
